Biotin and hair loss: the honest picture from a 2026 systematic review — who it helps, who it doesn't, and why most people taking it are wasting their money.

In brief, biotin, vitamin B7, is the most widely purchased hair supplement in the world. A May 2026 systematic review, the most comprehensive review of biotin and hair loss published to date, searching through December 2025, found no strong evidence that biotin supplementation promotes hair growth in people without a true deficiency. True biotin deficiency causing hair loss is uncommon in people eating a very diet. If your biotin levels are normal, taking more biotin is unlikely to benefit your hair. Here is what the evidence actually shows, and the one scenario where it genuinely matters. Biotin is a reasonable thing to believe in. It is a water-soluble B vitamin essential for keratin synthesis. And hair is made of keratin, the biological connection is real. Biotin deficiency causes hair thinning and loss. If you correct a biotin deficiency, hair health can improve. The supplement industry took this logic and extended it. If deficiency causes hair loss, supplementation must improve hair. It is a common reasoning error, and the clinical evidence has been patiently correcting it for years. A systematic review published in MDPI Dermatology in May 2026, following Prisma methodology and searching PubMed Medlin through December 2025, found that while overt biotin deficiency can be associated with hair changes, clinically meaningful deficiency is uncommon in individuals consuming a balanced diet, and published findings on biotine status and hair loss populations are incons. I stent. No strong evidence supports using biotin supplements to promote hair growth in non-deficient people. Does biotin help hair growth? What does the clinical evidence show? The evidence question is not, does biotin affect hair biology? It does. The evidence question is: does supplementation in people with adequate biotin levels produce measurable hair benefit? On that question, the data is consistently disappointing. A 2024 crossover randomized controlled trial published in the Annals of Brazilian Dermatology, Valentinal, Universidade Estadual Paulista, directly compared 5% topical monoxidil, 5 mg oral biotin daily, and the combination in men with androgenetic alopecia. The study found that biotin alone did not significantly outperform placebo for hair growth. Monoxidyl significantly outperformed placebo. The combination of monoxidol plus biotin did not significantly outperform monoxidol alone. Biotin added nothing to the monoxidyl effect. It was not synergistic, it was inert. The studies that do show positive biotin results almost universally involve either confirmed deficiency, multi-ingredient supplements where biotin cannot be isolated as the active, or conditions where biotinidase deficiency, a genetic enzyme defect, is the underlying cause. In a study examining children who experienced improved hair growth after taking biotin supplements, all had an underlying condition that caused a biotin deficiency. Outside of deficiency, there is no strong evidence to support taking biotin supplements for hair growth. Who does biotin actually help? There are specific populations where biotin supplementation is genuinely indicated for hair health. True biotin deficiency, rare but real. True biotin deficiency produces a characteristic syndrome, diffuse hair thinning, brittle nails, scaly red rash around the eyes, nose, and mouth, and neurological symptoms including fatigue and depression. This is genuinely uncommon in people eating a varied diet. Most people obtain adequate biotin from food, eggs, nuts, seeds, legumes, fish, meat, and whole grains all contain meaningful amounts. If you eat a reasonably varied diet, true deficiency is unlikely to be the driver of your hair loss. Raw egg white consumption, a specific and avoidable cause, raw egg whites contain avidin, a protein that binds biotin in the gut and prevents absorption, causing biotin deficiency over time. Cooked egg whites denature avidin, eliminating the binding. This is a niche cause but worth knowing. If you consume large amounts of raw eggs, some fitness regimens, you may be inducing biotin deficiency. Cooking the eggs or supplementing biotin addresses this directly. Long-term antibiotic use and certain medications. Long-term antibiotic use depletes the gut bacteria that produce biotin as a byproduct, potentially reducing biotin availability. Prolonged use of anticonvulsants, valprote, carbamazepine, and isotretinoin has also been associated with reduced biotin status. In these specific contexts, biotin supplementation may be warranted, not as a hair growth supplement, but as a nutritional repletion strategy. Biotinidase deficiency, genetic diagnosed in childhood. Biotinidase deficiency is an inherited metabolic disorder where the enzyme that recycles biotin does not function properly. It presents in infancy and is typically identified through newborn screening. In this population, biotin supplementation is a genuine medical treatment, not a wellness supplement. This is not a condition that presents as adult hair thinning without prior diagnosis. No Effect 2024 crossover RCT, Valentim et al. Biotin 5 mg daily did not outperform placebo for hair growth in men. Monoxidil significantly outperformed both biotin and placebo. Uncommon true biotin deficiency in people eating a balanced diet.
